We are excited to introduce our new Patient Portal. Using the portal, you can access you medical record and perform other tasks such as update your health history information or send a message to our office. You will initially need an invitation from our practice. Please call or email our office to set up your online patient account.

If you are a new patient or have not recently visited us, you will need to complete our patient questionnaire (aka My Health) in the Patient Portal. If you did not receive a user name and password, please contact us so we can assist you. Completing your patient questionnaire before your appointment will expedite the check in process.

The Patient Portal is best accessed on a computer. Pages may not load properly on a smartphone or tablet.
