Insurance We accept most insurance. If your insurance plan requires a referral to see us, it is your responsibility to obtain the referral from your primary care physician's office prior to your appointment. Please contact your insurance carrier to determine your specific benefits including any deductibles, co-insurance, and out-of-pocket expenses. Please verify that our physicians are in-network providers on your plan. If we do not participate with your insurance carrier, you may still see one of our physicians out-of-network; however, you may have a higher copay or coinsurance. Our office staff will be happy to assist if you have questions. As of 3/1/11, we participate with the following plans: Aetna Payment Information Co-payments and payment for services not covered by your insurance will be due at your visit. For your convenience, we accept cash, check or all major credit cards. We will also file your claims for all surgical procedures. Remaining balances and deductibles will be billed to you. Cancellation Policy If you make an appointment and cancel it less than 24 hours from your appointment or fail to show for your appointment without any notice, you may be charged a cancellation fee of $75 or $25 if you are a new patient or a returning patient respectively. |
